Refund Policy: Client may request a refund if they no longer need the service up to 24 hours before the agreed upon appointment date and time.
Rescheduling: Client may reschedule appointments with 24 hours&#8217; notice as needed.

Please provide us with the following information for your DNA test collection. For legal cases, please be sure to input your information exactly as you want it displayed on your notarized report.

We’re going to produce the report based on the information provided here. For legal cases, we’re also going to request IDs from each participant. A photo is sufficient for participants who don’t have photo ID.
